Installation Problems / Install Problems. What to edit in my index.template.php?

Im trying to get this installed on the Orange-LT theme.  I usually use the package parser for mods but am unable to find the changes to edit in my index.tempalate.php.  I'm thinking maybe I have a mod that changed something as even the installation on the default theme fails on the index.template.php.   If anyone could assist me I would really appreciate it.  I attached my index.template.php if that helps.  Thanks!
